The following is the form we will get all team members to sign when we visit you at school in Term 3. You may wish to use some of the language as a model for any permission notes/release forms you need to create for students and teachers.

NSW LearnScope 2007 Release Form
Each year LearnScope team members participate in a wide range of team and state-based activities. During these activities we make efforts to record a range of media that can be used for reporting, promotion and to share your learning with others.
To ensure that we conduct this process in an ethical manner we require each team member to sign the following release form.

I give permission for my image and voice to be used by my organisation, LearnScope and the Flexible Learning Framework for educational, reporting and promotional purposes. I understand that this may include photos in publications and shared on websites, edited video footage, and MP3 recording and podcasts.
